Living With Dementia in Nursing Home Dissertation
Living With Dementia in Nursing Home - Dissertation Example ENGDAHL (2013) explains dementia as a kind of illness that is caused by brain disorder. It is caused by failure of some parts of the brain to function properly. The actual cause of failure of the brain functioning is not yet established by the scientists. There is not specific cure of dementia, while its severity intensifies with age. Victims of dementia have problems with thinking and remembering things clearly. Communicating and day to day chores among the victims is also a problem. As elaborated by ELSEVIER (2015) in the book - Confusion and dementia, people living with dementia are also prone to depression, mood swings and cases of aggressiveness. Due to the aspect of forgetting, people suffering from dementia may easily get lost or forget direction, hence going wandering. Over the years, scientists have been studying on the actual cause of the disease, but all in vain. Lack of the appropriate cure of dementia has been a major challenge in the health sectors. However, nursing department has created hope to the issue. The home nursing department has come up with various ways that can be implemented to mitigate the effects of dementia to the victims. These can be summed as living well with dementia in nursing home (CLARKE 2011). The nursing department explains how these patients worsen with age, and the need for personal care and close monitoring of the victims to ensure that their lives are not endangered.

Public Administration and Apply Behaviorist Theory to a Work Issue - Essay Example Each of these concepts describes the change in a personââ¬â¢s behavior and the reason for such changes. The conditioned response relates to an activity that results due to a different incident. The next concept of behaviorist theory is aversive behavior. (Plaud, 1998). A person might exhibit a certain kind of behavior in response to a particular situation. This might be due to the disinterest towards the situation. Reinforcement aims at controlling the negative consequences of a behavior which helps in improving a personââ¬â¢s behavior. Consequences are one among the most important concepts of the behaviorist theory. A consequence generally arises from a particular decision or an action. Consequences generally turn out to be positive or negative depending on the behavior of the person. (Dewey, 1999). The situation too plays a major role in deciding the consequence of an action. American poet Jack Kerouac Essay Example for Free
American poet Jack Kerouac Essay American poet Jack Kerouac has been classified as the originator of the Beat Generation. He is known for his spontaneous confessional style, as well as his connection to Buddhism, Allen Ginsberg, and American self actualization. His rejection of mainstream 1950ââ¬â¢s society is his trademark, and the core ideal behind the beat-nick culture he helped spawn. Jack Kerouac was born Jean-Louis Lebris de Kerouac, to French-Canadian parents, in Lowell, Massachusetts. Kerouac was raised fluent in French, and didnââ¬â¢t start learning English until he was six years old. While attending Boston College and Columbia University, he demonstrated extraordinary athletic ability in the game of football, until he broke his leg. After losing his football scholarship, Keroauc moved to New York. There he met the makeup of the group that would later be known as The Beat Generation poets. This group consisted of: John Clellon Holmes, Allen Ginsberg, Neal Cassady, William S. Burroughs and Herbert Huncke. He left the company of these men to join the United States Navy in 1943, from which he was discharged during WWII, for the psychiatric measure of having an indifferent disposition. This is suspected by many to be the spark that inspired him to write his first work The Town and City, as well as On The Road. Many of Kerouacââ¬â¢s works were often rejected upon first submission to publishers. He was often overlooked, due to his experimental style, and socially conscious views. Not to forget, his excessive tendency towards drinking. This obsession with alcoholism is eventually the cause of his death in 1969, when he dies of internal hemorrhaging due to cirrhosis. Jack Kerouac has a very distinct and original style. Upon first evaluation of his style, his work might be deemed as cliche, or showy self loathing libation; but it must be remembered that, Kerouac was one of the first to adopt his free flowing form of poetry. He is also noted for connecting his poetry to the beat-nick poets. Kerouac is most commonly referred to as the King of the Beats and the Father of the Hippies. This is largely due with the fact that his poetry is most immediately inspired by Jazz music, specifically bebop by Thelonius Monk, Dizzy Gillespie, and Charlie Parker. He compiled this Jazz influence with a love of Buddhist studies, specifically Gary Snyder. The name he dubbed his new found form is Spontaneous Prose. It is often compared to the literary technique known as stream of consciousness, which is often deemed as a literary mirror of a writerââ¬â¢s thought process. While writing, Kerouac took the first thought or line that came to his mind and went with it. This technique is most recognizable in his novels Visions of Cody, Big Sur, Visions of Gerard, On the Road, and The Subterraneans. The core concept this method revolved around is a breathing technique very similar to that of Jazz, or Buddhist meditation. It is most evidently represented by a dash mark in place of a period. The practice of improvising words based on implied or inherently inferred meanings was another aspect of Spontaneous Prose. . The core concept this method revolved around is a breathing technique very similar to that of Jazz, or Buddhist meditation. In his statement on Belief Technique For Modern Prose, in his top five pieces of advice Kerouac says, 1. Scribble secret notebooks, and wild typewritten pages, for yr own joy 2. Submissive to everything, open, listening 3. Try never get drunk outside yr own house 4. Be in love with yr life 5. Something that you feel will find its own form (Kerouac) This is more the literary technique of an experimental thinker than actual literary scholar, or poet, during this era. Despite this, his style later becomes the format taught to most contemporary writers aspiring to be professional. Kerouac never really edited his poetry. In fact, the majority of his work was edited by fellow Beat Generation pioneer Donald Merriam Allen, who also served as an editor for Ginsberg. Acclaimed photographer Robert Frank made a short film personifying many Beat ideals called Pull My Daisy, in which Kerouac is scene reading his work in the films opening. The film itself is also based a play written by Kerouac. The Beat Generation is an uprising that formed mostly on some of Americaââ¬â¢s best-known college campuses in the mid to late 50ââ¬â¢s. It influenced students in the early 60ââ¬â¢s to seek hard hitting answers to some of the complex questions posed to their generation. Addressing ideas like the coexistence of wealth and poverty, along side with racial prejudice resulted in the beat-nicks forming what would be considered the American New Left. An example of this is the Students for a Democratic Society (SDS); which founded at the University of Michigan in 1960, it became one of the key organizations formed in reaction to Beat ideals. The core belief of this student body was that Americans should practice direct democracy as apposed to representative democracy. This stemmed largely from the strict nature of Universities during that time in which students felt they were just numbers in a system dictating their existence. These students were also known for being supporters of the Black Power movement. These groups are also historic for being the most visible resource for developing protests against the Vietnam War. This New Left was the child that the Beat Generation gave birth to, and its influence on American culture can be credited for many of the sociological changes in our society today. Jack Kerouac, along with the rest of the beat-nick poets, can be credited as well. They were the flame that sparked this generational chain reaction. In sum, when I read Kerouacââ¬â¢s work, I canââ¬â¢t help but notice that many ways his style has been adopted by American culture. The ideal of the creative minded young artist bathing in libation and pondering death and social inadequacies is the main image adopted by the majority of Rock n Roll groups today. The majority of these bands probably donââ¬â¢t even know of Jack Kerouac. Iââ¬â¢m not saying he was the first poet, or artist, to get drunk and create; but, I do acknowledge the ability of his free will to inspire so many others around him to create the cause to social change. He is pioneer and a leader, because he is fallowed beyond the grave. Work Cited Anderson, James C. Robert Frank. Microsoft Encarta 2006 [CD]. Redmond, WA: Microsoft Corporation, 2005. Jack Kerouac. Prevalence of Inappropriate Behavior With Schizophrenia
Prevalence of Inappropriate Behavior With Schizophrenia Sexuality is basic human need. This need is present at every developmental stage and as age increases, it become more dominant. According to Freud if person fixated at one stage or its needs repressed then in later life it comes in his behavior (Townsend, 2006). According to Muslim Public Affairs Council (1996) sex and sexuality are consider taboo subjects in Pakistan. Our culture also doesnââ¬â¢t allow talking about these issues openly before marriage and consider legal to satisfy sexual desire only after marriage. Religion also doesnââ¬â¢t support to discuss these matters openly. It is important to discuss on this topic in mental health because if we donââ¬â¢t discuss on sexuality, the patient may start expressing his thoughts in unacceptable manner which is against our norms of society and it is called as ââ¬Å"sexual disinhibitionâ⬠. The study uncovered that 26.7% of persistent schizophrenic patients had huge obsessive enthusiastic indications, with a high prevalence in the age set underneath 35 years. Obsessive habitual side effects were more serious in patients with term of sickness more than 5 years. The obsessive impulsive indications were more common around paranoid schizophrenics (Hemrom et al, 2009). A 33 yr. old female admitted in Karwan-e-Hayat with complain of schizophrenia. On assessment I found her depressed and prepared to give teaching on coping mechanism. While giving teaching to her on coping strategies she suddenly started telling her sexual thoughts, desire and feelings to have a sexual intercourse. Even she was ready to have an intercourse with his divorced husband and wanted to get married. When she was telling this I was quiet, anxious, uncomfortable, and not confident to discuss. Once I thought that I was not able to assess her problem for which she needed help. In schizophrenia obsessive compulsive symptoms that of being contamination, sexual, and aggressive thoughts is the positive sign. In a research it has been found that 10% of schizophrenic patient had these obsession symptoms (Hemrom et al, 2009). Sexual obsession is common in schizophrenic patients and relates the DSM-IV criteria of schizophrenia and OC (Bancroft, 2008). Who encounter such a sort of unadulterated fixations (i.e., fixations that are regularly portrayed by the absence of unmistakable impulses) assess unpleasant considerations as unsafe and excessively significant and, hence, fight to control their beliefs (Dellââ¬â¢Osso et al, 2012). The patient was suffering from schizophrenia so having sexual obsession could be the part of her disease process as Turkcan et al. (2007) reported that 16% of patient had obsessive compulsive symptoms with schizophrenia. According to Freud (1961), psychoanalytic theory describes formation of personality through five stages of psychosexual development. Fixation of libido (sexual drive or instinct) at any stage of development certainly results in psychopathology (Townsend, 2006) and fixation at any stage will also influence the behavior of person in later life (McLeod, 2008). As in case of my patient, she was divorced due to which her sexual desires were repressed and unable to fulfill them. It results in fixation of sexual needs due to which her sexual needs are at extreme level that, she have intense feeling to have intercourse and have sexual thoughts, which may change into behavior at any time. On the other hand, literature shows that some antipsychotic drugs induce sexual disinhibition which includes risperidone and quntipien (Lam et al, 2007). Risperidone side effect is that it increases sex drive and also decreases sexual function in some cases. Some cases have been reported which shows the effect of risperidone in increasing the sexual obsession compulsive thoughts (Basil et al, 2002). So, my patient was also taking risperidone it may be one of the factor for having compulsive thought. Whereas, there is also literature support for medication that decreases sexual desires such as one of the late study evaluate that second generation antipsychotic i.e. risperidone, olanzapine, quntipien, and haloperidol is the biggest study to date to assess sexual dysfunction and reproductive side effects (Kelly Conley, 2004). Therefore, it is important to take alternative medications to treat these symptoms. Whereas various late medicine studies and narrative case reports have indicated an adjunctive particular serotonin reuptake inhibitor (SSRI) may be a convincing medications to treat OC in schizophrenia. Patients getting clozapine and other atypical antipsychotics as their support medicine ought to be thoroughly screened for new onset or compounding of prior OC indications (Hwang et al, 2006). Although my patient had intense thoughts and desires to have intercourse, so with the pharmacological management it is important to do cognitive therapy because patient only have thoughts, but her thoughts can be change into behavior. It is important to change her thoughts through cognitive therapy. There are different strategies through which inappropriate behavior of patient can be change and these are discussing below. Cognitive therapy helps dependent upon perception, and all the more particularly, the particular judgment evaluation by a single person of an occasion, and the feelings or practices that come about because of that examination. Cognitive techniques include identifying and modifying repeated thoughts (thinking errors) and schemas interior values (Townsend, 2006). The goal of cognitive therapy is to change irrelevant beliefs, faulty way of thinking, and negative self- statements that cause behavioral problems (Stuart, 2013). Cognitive therapy focuses on controlling and reducing obsessive compulsive behaviors by behavioral strategies for managing symptoms, thoughts, beliefs and feelings are perused and examine for relevance and validity. Behavior modification system is dependent upon the precise examination and requisition of fortification. Support is the procedure by which new reactions are obtained and existing ones are fortified (Jessor, 2013). Moreover, motivational interventions can be done to develop patientââ¬â¢s motivation to change. By asking open-ended question can help to identify patient agenda. Affirming, reinforcing positive statements, and hopeful can emphasize that change is possible and it will also increase the self esteem of patient which will help him and motivate to change (Stuart, 2013). Role modeling can be done to teach the behavior. Lifestyle change to help an individual identify high-risk situations, change cognitive distortions or faulty thinking, and cope with stressful or high-risk situations that may trigger relapse (Townsend, 2006) Furthermore, nurse role is important in recognizing these issues in mental health. As, in my patient scenario it was my lacking that I was unable to recognize patient needs correctly so as nurse it is important to asses patient needs rightly and to do interventions accordingly. For this nurse should be competent in communication skills, confident to talk on this sensitive topic. She should have complete knowledge about the topic, positive approach and effective communication style can greatly improve the interaction. An approach that rises strengthening and self-governance ought to be utilized and the suitable health-advancement messages through consultation for individual client (Gott et al, 2004). All in all, prevalence of inappropriate behavior with schizophrenia is high; therefore as nurse it is important to asses patient needs and play the role of counselor, advocator, and as a care giver in giving awareness to families about patient disease process. Family and health care support also helps patient to cope his situation. In mental health it is important to modify the behavior of client according to needs through required techniques.

Representations of Women in Ike Oguine A Squatters Tale :: Squatters Tale Essays
Representations of Women in A Squatterââ¬â¢s Tale Women can be perceived or looked at in many ways. They are depicted not only as mothers, but also as friends, companions, and even prostitutes. Todayââ¬â¢s society has a variety of images of what they feel women should be and what they actually are. Likewise in Ike Oguineââ¬â¢s A Squatterââ¬â¢s Tale, women are portrayed through various roles such as mothers, girl friends or companions, and prostitutes to reflect the society. First, mothers are backbones of the family. When hard times are experienced, they are the ones who keep the family together. Women provide more than financial support to the family. They are the sense of hope and encouragement to keep the household running smoothly. In A Squatterââ¬â¢s Tale, Obiââ¬â¢s mother is the one who, even in hard times, keeps holding on trying to get through the difficulties. The love she has for her family is what gives her the strength to give all she can to her family. When Obiââ¬â¢s family move from their Yaba house to a three-bedroom flat in the outer reaches of Isolo, as a result of the sudden retirement of Obiââ¬â¢s father, their struggles for survival increase. Obiââ¬â¢s mother would drive everyday from Isolo to her shop in Yaba. The drive she endures everyday is very long and tiring. Obi knows his mother is enduring a lot for the family as Oguine establishes this sense of hardship by stating in Obiââ¬â¢s words, From our new home my mother had to do a hellish drive to her shop in Yaba every morning (leaving behind two disabled men ââ¬â my father and me ââ¬â brooding in the living room all day in front of the shiny black Sony Trinitron 21â⬠TV I bought when I was treasury manager in BTF) and another hellish drive back every evening through the most chaotic traffic in the whole of Lagos. (113-114) The struggle of the drive alone is hard for Obiââ¬â¢s mother. When her car breaks down after ten years, the battle against hardships is about to be lost, because, in addition to working extremely hard, she is being put through hell. Obi realizes what the struggle is doing to his mother the day she comes home after her car has broken down.
